To find exactly the value you need to know, no matter if its armour or weapon, with this percentages and a calulator you can see, what stats yout item would have in wich upgrade from +1 to +16. All you have to do is take the ground value of defense (armour) oder attack (weapon) and multiply with the percents i give you:
+1: 5.85 +2: 11,70 +3 : 18,62 +4: 26,06 +5: 33,51 +6: 41,49 +7 : 50,00 +8 : 59,04 +9 : 68,62 +10: 78,72 +11: 110,11 +12: 125,00 +13: 140,96 +14 : 157,45 +15: 175,53 +16 : 194, 68 For example: The lvl 37 Titanaxe has attackvalue of 149, if you want to see the bonus at +12, take 149 * 125/100 = 186,25 = 186. Keep in mind that +15 adds an extra bonus of 75 on physikal weapons or +100 on armour that is NOT included in this formula!
